What hospital do I go to when I am a patient of Elder Care of West Michigan?

You may go to any hospital of your choice in the area. Elder Care of West Michigan has live-time access to records for both Metro and Spectrum Health Hospitals. Records can also be obtained by fax from Saint Mary’s hospital.

If I go to the hospital or see a specialist, where do I have my records sent to keep Elder Care of West Michigan updated?

Please ask for your records to be faxed to the our dedicated fax number at 855.548.4481.

If I have home health care provided through a company, how does Elder Care of West Michigan coordinate with my home care services?

Elder Care of West Michigan has a close working relationship with numerous home health care companies. We communicate directly by phone and fax with your nurse, physical therapist, occupational therapist, and speech therapist in all circumstances.
